20 KB is a very small budget for an image. It is the sort of limit set for a signature or a thumb impression rather than a photograph, and a full-colour photo almost never fits at 20 KB while staying recognisable.

The tool works in two stages. First it lowers the JPG quality, step by step, testing the real file size each time rather than guessing. If quality alone cannot get there and you have left "reduce the dimensions too" ticked, it then shrinks the image by 15 percent and tries again, repeating until the file fits or the picture is as small as it can sensibly go.

The quality floor protects you. Without it, a tool chasing an impossible target would keep degrading the picture until nothing was readable. Here, if 20 KB cannot be reached without passing that floor, the tool stops and tells you the closest size it managed.

If you are compressing a signature, convert it to black and white first with the brightness tool. Two-colour images compress far better than full-colour ones, and 20 KB becomes easy to hit.

My photo will not reach 20 KB.

For a colour photograph that is normal. Tick "reduce the dimensions too" and the tool will make the image smaller on screen as well, which is usually what gets it there. If it still cannot, the result tells you the closest it reached without making the picture unreadable.

Why is the quality dropping so much?

20 KB is a tiny amount of data. Lower the quality floor if you genuinely need the smaller file, or raise it if readability matters more than hitting the exact number.
